Suspected Adverse Events & Product Complaints
Should you need to report a suspected adverse event or a product complaint, please consider providing the following information to help us investigate quickly and effectively:
- Your contact details (including email and telephone number if possible) – this allows us to follow up with any questions and resolve the issue promptly.
- Batch number of the product in question.
- Photograph of the product if your enquiry relates to a product complaint.
- Sample retained – let us know if you have kept a sample of the product.
- Yellow Card report details – if you have reported the incident via the MHRA Yellow Card system, please confirm the date of the report and whether you agreed to share your contact details with us.
